AMI Health Solutions Massage Consent, Screening and Indemnity
1. Nature of Treatment
AMI Health Solutions provides massage services for relaxation, general wellness, muscular relief and recovery support, where appropriate. Massage treatment is not a substitute for medical diagnosis or medical treatment.
2. Health Disclosure
Clients must provide complete and accurate information about their health, injuries, pregnancy status, medication, allergies, skin conditions and any other factors that may affect treatment. AMI Health Solutions may modify, postpone or decline treatment if it is not reasonably considered appropriate or safe.
3. Ongoing Consent for Regular Sessions
This form serves as a general intake and ongoing consent form for massage services requested from time to time. Clients agree to inform AMI Health Solutions before each future session if there has been any change in health status, medication, pregnancy status, pain, injury, skin condition or any other relevant factor.
4. Treatment Boundaries and Client Rights
Massage treatment is strictly professional. Appropriate draping will be used and only areas reasonably necessary for treatment will be exposed. Clients may request adjustment of pressure, ask that an area be avoided, ask questions, or stop treatment at any time.
5. Possible Responses to Treatment
Clients understand that massage may involve pressure, stretching and manual techniques and may result in temporary tenderness, light soreness, sensitivity or fatigue after treatment. No guarantee is given regarding specific results or outcomes.
6. First Aid and Emergency Response
In the event of illness, injury or an unexpected reaction during treatment, AMI Health Solutions may administer basic first aid and, where reasonably necessary, contact the emergency person listed on this form and arrange further medical assistance.
7. Privacy and Information Handling
Personal and health information supplied in this form will be collected and processed for treatment screening, service delivery, record keeping, communication, safety, legal and administrative purposes. Information will be handled confidentially and used only for legitimate treatment, business, legal or safety purposes.
8. Liability and Indemnity
To the fullest extent permitted by law, clients accept the ordinary risks associated with massage treatment after disclosure and consent. AMI Health Solutions and its therapists will take reasonable care in providing services, but shall not be liable for injury, loss or damage arising from inaccurate, incomplete or omitted information supplied by the client, or from the client’s failure to disclose relevant changes in condition. Nothing in this form excludes liability for unlawful conduct or gross negligence, nor removes any rights the client may have in law.
9. Governing Law
This agreement is governed by the laws of South Africa, and any dispute shall be subject to the jurisdiction of the appropriate South African forum or court.
